Neck is straight, and does not show signs of bubbling (common with that type of coating).
I believe the nut has been replaced.
Regarding the preamp it currently works in mono with the bottom output jack, but in stereo there is a huge quantity of noise. I have replaced the opamps with no changes and given how spaghetti that preamp is I really don't want to mess around. But I also have schematics.
At the moment it has Hipshot Ultralite tuners and Schaller straplocks mounted, it will be reverted to stock components.

Comes in a SKB hard case.
