One more thing, I rechecked the serial number, and the actual date of manufacture was June 1997 for the cabinet. The amp was 2003.

I wired it just as Wasnex suggested, and used the original internal wiring to form parallel jumpers. The jack plate is indeed marked for + and - as well as upper and lower. I was also pleasantly surprised to find that the speakers in this 2003 SLM cabinet are Eminence.
