Controlling flange rate

Not that I am aware of. The Kickdisk and Third hand allow you to use your foot to control the knob on one of your pedals. A multi like and M9 or B2.1u will have some parameters that can be controlled with an expression. I don't know off the top of my head if their flangers will do THAT sound to your satisfaction, but they should have some control over those parameters.

M9 definitely has expression control with a separate pedal. I don't know which parameters a B2.1u will control.

You would want a Stereo In / Stereo Out to do it in high quality....

Live is tricky, but doable with a good soundman at the helm.

No soundman?....very difficult without the right gear.

If I was going to have the effect in a song I'd prefer it to be a single instrument. I was just using that song as an example of controlled flanger effect.
 
You might find a pedal that you can just put a giant knob on that you could control with your foot, or use an MXR with one of the rubber boots on the knob for that purpose....(or put the boot on another pedal). Sounds tricky to me, and I'm too much of an oaf to pull it off, but you might be able to get it to work for you.....
