I have a phobia about forgetting to pack something, like cords, for a gig. Too many times I've pulled over to go through stuff. Never forgot something, but you never know.

Claustrophobia. I start sweating just at the idea of having to be in a confined space or even watching someone else in a confined space. I had to get an MRI years ago, before the open-MRI technology was widespread, that was one of the most horrifying experiences in my life.
