Hi, Could some please help to identify made of this bass pre? Wiring diagram would be nice too.

Yeah, that seems to make the most sense. My initial thought was that it looks a little big to have been in a bass, but if it was in that pedal, that makes more sense to me. The PDF has their company website, I would contact them directly and ask about it. If it is their work, they’ll have better answers.
