Yup, I have a similar setup... A dual mono output from my pre-amp mounted vertically on the tailpiece via a mono splitter patch cable. It's really nice to be able to run a personal amp and be DI'ed at the same time. Also, all the plug action is there instead of at the pre-amp; save the in-out traffic on that pre-amp output socket by using something easily replacable.

Hmmm, please correct me if I'm wrong, but it appears to be a 1/4" right angle split adapter, that allows another male 1/4" plug to plug into the black end... Basically a split adapter in an angle plug.

I've not been able to locate this adapter for sale online so it may be a fixed part of a proprietary cable or patch-cable.

...I knew someone would be curious...it's a Neutrik Timbreplug...great addition to my line...it has a 4 position rotary switch that has straight thru and 3 positions of micro resistors in gradients of resistance...incredible unit...I run it in the position of least resistance...if I need more signal I move to straight thru...if I encounter feedback or need less signal I can move to an increased resistance...it does warmly darken the signal so if I'm looking for a different sound I have 3 settings to use...it's on the drivers side of the tailpiece so live access is right now...
changes the timbre of the instrument...they make one that has a silent auto mute for changing instruments under load...cool product...
 
Very interesting. Clearly my earlier impressions were wrong...

I see Amazon has the Neutrik ultimatePLUG which combines the silentPLUG and timbrePLUG in one product.

The descriptions indicate that these were somewhat targeted at guitar, do you find the gradient settings work as nicely with DB?

Also, do you use a DB mounted pre-amp?
