Installing a truss rod

Ok, so the last time I installed a truss rod and the neck settled I went to adjust it, the adjustment nut broke off. (I used an LMI double action btw). Is there anything I can do to prevent this from happening this time, was I supposed to turn the truss rod to a certain point before installing?
 
You want to make your adjustments *small*... start with no tension on the rod, and make no more than 1/4 turn, and then let the neck settle for several hours. If it needs more adjustment at that point, try 1/8 turns or so, letting it settle again between each adjustment
