Lakland 55-02 Preamp Issue/ Active Electronic Issue

Kind of long but I need some help here. Wondering if any Lakland owners or active electronic bass owners have seen this issue -

I bought a 55-02 from Lakland early Dec 17. I've replaced the preamp twice now. When I received it, right out the box, I played it for about an hour, heard a pop (sounded like the amp blew a fuse), no sound for about 30 seconds, then when the sound came back, there was no low end. I could turn up the bass knob to the flat/half way position but if I went past that (boost direction) it completely cut the volume lower than if I rolled all of the low end off. Every now and then the mid range sounded a little weird too. The bass was plugged into a TC Electronic Polytune Noir tuner plugged into a MXR bass octave deluxe into an MXR bass envelope filter into a Boss bass chorus. All of the pedals were powered with a Voodoo Lab Pedal Power Iso 5 plugged into a Furman surge protector. The amp I was using was a Mesa tube amp (not mine) with a 4x12. For the first hour everything sounded great. Sent it back to Lakland, and they replaced the preamp and returned the bass within a few days.

Seven months later, in July '18, after several gigs and rehearsals without any issue, the new preamp blew out. This time going through my personal rig - Mesa Subway d800/Mesa Subway 1x12 and 1x15, same pedals and power as mentioned above. Lakland sent another preamp and everything was fine up until November 18 or so. This time it's different - the low end is fine but the volume has to be turned all the way up to sound normal. If I back it off even a little, it cuts the volume drastically. I haven't reached out to Lakland again but will soon as the warranty is up in December of this year.

I play a Sadowsky 5 (early 90s model) through the same rig and have zero issues. Also, the Lakland works fine in passive mode.

Has anyone experienced anything like this? Would it be best to replace the pickups and preamp?
