Pedalboard mountable IEM mixer?

I am not sure if what I am looking for exists but I'm sure you guys will!

To keep the info short, my effects chain ends in a single 1/4" mono output (from a small 4 channel mono mixer mounted on the board) which I either connect to a DI to send to a PA system and monitors etc. or I use a mono to stereo cable (sending the mono signal to both left and right) which I connect to a headphone amp for IEMs when practicing on my own.

I am looking for a solution for when I would like to add other players/sources into my IEMs. I know (and own) that any small mixer would work but I would like to find something that would fit on my board.

I need:
- Minimum 1x 1/4" mono input
- Minimum 1x stereo (or 2x mono) input
- A way of controlling the volumes
- A "thru" for just the first 1/4" mono signal that is not affected by the volume
- Standard pedal size (or only slightly larger)

Would like:
- A DI output for the first 1/4" mono signal (again, not affected by volume)
- 9V powered

I am also open to removing my current 4 channel mono mixer from the board but then I would need 2x 1/4" mono inputs that have "thru" (and DI if possible).

Does anything like this exist?
 
There's nothing really that matches from Rolls. I guess plan B would be to run my mono output through a DI box before going into another small mixer to control IEMs.

I am guessing that Rolls would still be the best option for a small mixer with at least 3 mono inputs and a stereo output.
 
I use this setup:

The Rolls personal monitor station works like a 3 channel mixer that can be run in either stereo or mono, each channel having it's own volume control. I play my bass through "inst" channel, my vocal through the "mic" channel, and I run an aux out from our band's PA through the "line" channel. The Lehle isolates my bass amp from the PA, and prevents noise. The Headphone outputs run into my IEM transmitter.
 
I bought this off a friend who commissioned a custom job from SFX in the U.K.

Does everything you mention and the DI is impeccable on it (replaced my countryman type 10)
