The Source Audio EQ-2 has all the function you seek, but it's digital.
The Empress Para EQ deluxe/bass has all the analog bells and whistles, but it's not programmable.
The Free The Tone PA-1QB does not have HPF/LPF, but it's analog and can be programmed and controlled via MIDI.
The GFI System Enieqma is another EQ like the Source Audio, all the bells and whistles one could ever wish for in an EQ, but digital.
The Boss EQ-200 is also digital and it also lacks the HPF/LPF.
In your shoes, I'd either insist on analog signal and go with the Free The Tone and add a simple HPF/LPF Pedal like the Broughton, or
I'd insist on getting all the functions and go with the GFI.
....or I'd comb through the Internet until someone comes up with some obscure unknown builder who just released exactly what you're looking for ;-)
