Question regarding 4 lug output jack install

So I recently bought this "KAISH" output Jack that has 4 lugs.
My previous output Jack only had 3 lugs. So I'm not exactly sure how to wire up the 4 lug OR how to identify which lug is which.
Can anyone help me out?

HERE'S WHERE I'M AT :
I believe the green wire coming from the volume pot goes to the "TIP"
(the shortest lug in the center)

The black wire from the battery goes to the "ring"
(not sure which is the ring)

and then the black wire from the preamp + the ground go to the "sleeve"
(also not sure which is the sleeve)

ALSO.. not sure what to do with the extra lug.

I think it has two (2) sleeve contacts, and you can use either/both for your ground. I'd plug a TRS cable into it and check continuity between lugs and the other end of the cable.
