The helmet was fine- it had nothing to do with his hair, and the fact that it came off was incidental. The clasps AND the straps are designed to break away at a certain force so that the straps don't choke or further maim someone. That the helmet came off is completely incidental and really just indicative of the fact that he was hit very hard in the head.
The injuries that resulted in the cardiac arrest were the spinal and cranial injuries, compounded by the lung injuries sustained by the chest impact.
The helmet coming off was not a factor in his death.
Nothing about safety or "something needing improvement" was a factor here, either. No amount of changes could have prevented this. He low-sided in the wrong place at the wrong time, and that's it.
