I had them fall off in the past, with Fender-style buttons, which are horrible.
Also had a problem with the screw working itself out when I used those rubber washers.

Haven't had a problem at all since using the over-sized strap buttons.

It's probably not as much of a concern if you don't gig - just sayin'... IME, live performances definitely increase the chances of a strap coming loose as opposed to rehearsals or playing at home...

Back when that happened I had no knowledge of oversized strap buttons.

I'd be willing to say that if you had over-sized buttons installed it wouldn't have been a near-accident.

Oversized buttons are great.
Ibanez made a V shaped button in the 80s that worked pretty well, too. I think they called it a Dead End(?) strap button?

I use those rubber washers, they.work very well. And no additional screw/drill work required.

Put a rubber/neoprene washer over your strap. I have these on all of my strap pins.

I used "rubber washers" originally. For what ever odd physical reason, the upper/neck grommet, pulls off/stretches enough to mitigate security.

In my professional procrastinating posting, here are my findings of the "Diago Twistlock" Strap locks... 1. I waited (see above) long enough to find them unavailable on all the sites on which they were previously located. B. Found them, oddly enough, from a sole on this board (a bit higher price, but: supply & demand is a thing). III. Easy installation & application AND it is *most* comfortable & noiseless... Be well. Be happy. Stay in the Groove.

Thanks, but the Diago Twistlocks work well. (& I don't drink any longer)... The ONLY downfall of these that I've found is: does not work well when cable is tucked between bass & strap...

Or, buy a set of rubber strap locks and get four beers free. I think it sounds like a better deal this way.

I use rubber washers made for this use by "Ish". The strap locks are nice, but for me they're overkill. I do have them on a couple of basses, but only because they were installed when I bought them.

I'm not sure Ish is still in business, but I see that Fender now makes a similar strap retainer, and they're about $3 for four:

Ahh, good question. Epiphone Viola. Neck button on rear of neck & therefore...

I have Dunlop Dual design locks on all of my basses and my Gold Tone Electric Banjo. Banjo has strap button mounted at one of the neck screws (similar to Viola position). I have had no issues or concerns with the locks accidentally unlocking. I am sure you could figure out a way to "accidentally" do it, but seems difficult to me. They are not the easiest to unlock. You have to push on the button while pulling away from the guitar. The odds of you pushing against your body hard enough to unlock it while simultaneously pulling the strap the other way seem extremely low to me.

The only reason I have ever had a strap come loose with the Dunlops was not having them fully locked in place (happened once, and I was holding the bass pulling on strap because I thought it was loose... it was).

You can get a pack of 100 for under $10 if you look for bottle gaskets on amazon. If you combine them with oversized buttons you should be plenty secure.
