The most important/useful pieces in my "survival kit" have proven to be a one-spot and a set of guitar strings.

I play a minimalist rig with no pedals and I've never broken a string at a gig. However both of my guitarists approach life with a personality opposite my own, which means large complicated pedalboards and also never having a backup plan. On two separate occasions so far I have saved the show (and my own paycheck) with my magical survival kit.

My emergency stuff is basically for the other guys. 9v battery, capo, drum key. Of course some simple tools.
Stuff for myself beyond what op mentioned: hand washing wipes. All these outdoors gigs serving pig roast or chicken, but no running water! And tylenol/aspirin.
