I'm not sure my version of Adobe allows me to pull a single page from my RealBook3.pdf, but if it does, I don't know how to use it. But here are the chords:

Oh, I didn't realize it was a 2 page form. The 2nd page has 5 line, since you've got the first three, the last two lines are:

The "To Solos" and "To Ending" are written like a 1st & 2nd ending. The last two bars of the ending vamp.
