Schaller M4 Hipshot Xtender. Bought here for a project, but wound up going with Hipshot Ultralites instead. Bushing and washer included, but not the tiny little screw. Email Hipshot and they’ll probably send you one, if you ask nicely….

The single small fixing screw that screws into the back of the headstock, and keeps the tuner from rotating. Regular tuners use a pan-head screw, but Hipshot uses a countersunk flat-head screw, because a pan-head screw could potentially interfere with the detuning mechanism. There's a good chance you could find a suitable screw at your local hardware store. Or you could file down the head of a pan-head screw until it clears. Or email Hipshot. Or...
