i don't know anything about it, but i would guess that he broke a string at a gig and didn't have any spares to replace it so he only played the 3 remaining strings for the rest of the gig

That's one of the stories I've heard. The other is that when he got his first bass 2nd hand, it only had three strings on it and he never bothered to replace it for the longest time.
Gov't Mule has a song on their newest CD dedicated to him called "3 String George." That's what sparked my interest in it.
