The where I was going is that substance abuse is by definition a mental illness. I agree that she chose to pick up the drugs, as do all abusers, but once they do (either through forms of self-medication through comorbid mental health diagnoses, boredom, peer pressure, etc, it becomes a mental illness. Illnesses require compassion and treatment and research demonstrates that compassion fatigue and shame-based interventions do more harm than good.

I only included the *likely* dx for Jaco as a comparison to indicate that both bipolar disorder and substance abuse are considered mental health problems under the DSM and ICD. It fits the history better than the other subtypes, which is where most dx are based upon (with the addition of psychometric data, which is of course not available for Jaco), but is considered hypothetical until connected with the history. The scary thing is that most physicians make a diagnosis based on history and do you know how long it takes on average to do so... 15 seconds. If Jaco were alive today, of course more information would go into the diagnosis, as should inform your statement that Amy doesn't have a mental illness that drives the need for attention as well as the choice to use and keep using.

I respect CADCs and LMFTs and work with them a great deal, but I think that your level of anger towards this one addict is not helpful to you or your profession. I am NOT saying this as an attack, but I can say honestly, that if I knew of a CADC talking about someone with addiction problems as you have talked about this singer (who I've never heard one song from and have no interest at all in vilifying or championing), I would not refer to them for services for one of my clients. I'm sure you are a great counselor, and your qualifications speak well for you. I would encourage you to do the same. ;-)

Also, it's one thing to attempt to dx Jaco without knowing/meeting/evaluating him, but to go as far as to specify the subclassification is a little scary, me thinks. I'm just sayin'. ;)



While I'm not a clinical psychologist, I am a NCC, CACD, and CMFT, and I'm working on my LPC. I'm very familiar with the DSM and the ICD. (WOW that's a lot of letters!!!!!) :D

I am too, man. Sorry if I came across as harsh. There is an extremely high rate of comorbidity between MH and substance abuse. I have a difficult time acknowledging many (if not most) of the substance abuse cases as true MH disorders however, despite them being referred to as such in the DSM in such a black and white fashion.

I'm with you there man. There's an old psychology joke that speaks to this...

If you meet someone who doesn't have a mental illness, then they haven't been assessed thoroughly enough yet. ;-)

(it's a stupid joke, and I don't believe it, but it does speak to how black and white the polythetic diagnostic system is). I usually tell my clients that I don't believe in diagnosis (I'm much more inline with Thomas Zsaz) in that dx are just labels for behaviors that fall somewhere on a normal curve... but I have to diagnose something for insurance companies. :-)

Good luck with the LPC exam! I took it twice within a week because I wanted to make sure I passed in both Missouri and Illinois. I was supposed to get different versions of the test, but they were exactly the same. I remembered all the questions from the first test so I was able to finish it within about 25 minutes and they give you 4 hours. They thought I cheated until I explained it! ;-)

Yeah, I've heard the joke, and there's (unfortunately) a degree of truth to it. Especially with children, where irate parents are quite good at coercing a dx from the doctor, even though many times familial issues caused the behaviors in the first place! :(

Thanks on the good luck. I actually passed the exam already, however. I'm just busy accruing my 2,000 hours now. I recently moved to CO from PA. It was only 1,000 hours required in PA. While I'm all for intensive training, etc., I was bummed when I found out I owed an extra 1,000 hours! :atoz:

Strychnine? Far out.

In a town near to me there has been a spate of illegal immigrants converting houses to pot growing concerns; and they've been spraying the immature buds with paint to get that frosted sparkle, more weight and higher prices.

Regarding Jaco/Amy, I would think that the lifestyle of a serious drug user & musician (or any demanding occupation) would tip a lot of people over the edge into mental illness through stress & fatigue, when ordinarily they would never have such episodes.
