Back tension/pain

I'm not a doctor, but the first and most obvious thing that comes to mind is posture. One possibility that might explain why it happens both sitting and standing is that you have a bad habit of constantly leaning over so you can look at the fretboard rather than keeping your back straight back. Are you guilty of that?

Um.... Stop doing that?

Seriously.... I suggest that you spend a few practice sessions focusing on your posture. Do something simple that doesn't require you to pay attention to your left hand so you can focus on your posture instead, and then gradually work your way back to your normal routine.
