Can anyone ID these pickups and pre amp for me ?

I got these in a project Bass that was trashed.I would love to put these back in service...any ideas?
Also a wiring diagram would be most helpful.

Might have to bump this one a few times...

The opamp on the pre looks like it says LM 358N, but it's hard to make out in the photo. You may want to research that exact opamp (whatever it is) and see if it came stock on any onboard preamps. Just a thought.
