D string problem EXTREMELY FRUSTRATING!

I have a sting ray I just bought some DR black beauties. This has always happend through 3 sets of strings. Im stringing up and its all fine until i get to D. When i put in the string in the tuner and tighten the strings goes up a certain amount and just slips right out of the tuner causing in to detune. And this will happen until the string slips out. Im very very angry can some one help me by pass this.
 
I actually had an identical problem with a set of red devils a little while ago... on the d string. I realized that i had been cutting the ends of the strings too short and too much tension was being exerted on the bend of the string down into the tuner hole. I now leave more slack and give it 2 or 3 wraps around first.

Really pissed me off because that was a good set :mad:

i have a lot of slack. I never cut to short. (I learned the hard way) But i threaded it through. again so its making a D shape. * In the hole, around 180 dgr, and through the center again. Almost making a knot. So solved for now. We
ll see what happens.
