D string won't tune

Ok so I just replaced the strings on my Schecter, same D'addario medium gauges as before, and I'm coming to a problem with tuning my D string. I can get the string a little bit above a B before something up in the machine head pops...Did I break the head? Or what else is the issue?
 
Sounds to me like the gear is slipping in the tuner. May be due to it being torn from age or a defective part. You best option is to find a replacement before your next gig. A bum tuner can be a big pain in the rectum come show time.
 
Heh, I think the next gig I have will be in a future life with how slow this band is moving..but I digress...
Had a problem with that tuner since I bought it..but always used to be able to tune the string just fine..knew I shouldnt have bothered
