i have a powered little 6 channel behringer mixer, what do i need to get me started with a simple IEM set up. basically what i am looking for is a send from FOH with a full mix and my bass, at the level i want it at.....dont know squat about in ears or the componants and dont know what i need, help me out guys.....

If you are wanting it for gigs...use the Mixer you have. Take an xlr in from FOH to a channel and ask for levels like you would a wedge (except you would have an overall master for yourself) and then take a parallel out of your bass (Presumably from the DI to FOH) and put it into another channel on your board. Now you have your full mix with its own master and a separate level control for your bass...
I have my bass monitor return from FOH into a 6 channel mixer. I also have the vocals into my mixer using mic splitters and if I need keys or guitar, I can run from the parallel outputs from their DI boxes. I hookup my ears to the phones jack,can equalize all volumes and have the 3 EQ options as well. Works for me in a church setting.
Sorry, yes the headphone jack, as I use a Samson MDR8. I did this way because I just wasn't getting the right monitor mix I wanted, using a Rolls 351 headphone amp, but switched to this current setup and love it.

Finally got it in a .jpg file format. Simple hookup, can control all vocals, my bass and any other hookup needed, volumes via the channel faders, or master faders or headphones knob . The bass signal is fed from the FOH mixer with all effects added, can also add effects that is in my mixer. Figured it out for my needs, may work for you.
I'm using a pair of Sennheiser HD280 Pro headphones. I'd rather use them than IEM for right now, since I'm usually sitting or standing in one place while playing at church. I have used them at gigs along with the Rolls 351. Very comfortable for me, but YMMV.
I bet it sounds good but I'm standing and moving back and forth some. I think they may fall off.

I keep one of these on my pedal board for venues where it's difficult to get a wireless
IEM signal...then I just take a line feed from a wedge into my box and to my IEMs.
It works extremely well.

shure se215, yes they are cheaper in ears but two other guys use them in our band and absolutely love them! they have better bass response than the 315's and have awesome olive tips which seal really well. best of all they absolutely will not fall out.
as far as the rest of your set up, that is what my current church is using. i run a monitor signal into channel 1 and then my bass parallel out of my sansamp into channel 2. from here just get a 1/4 inch to 1/8 inch adapter for the phones jack on the mixer and then plug into the that with your iem's. eventually if you want to go wireless all you have to do is send your phones signal from your mixer into a wireless transmitter and then youre wireless!
