Double Bass Joseph Jan Dvorak bass ID help?

Anyone able to decipher these numbers?
Greatly appreciated if anyone cares to assist!!

I'm guessing the hand written numbers are when repairs were done. The bottom number looks like July 12, 1934.

Also, if it's an upright you might have better luck in the double bass side of the forums. The mods can move the thread if needed.
 
It's a Czech bass, made by a company with long tradition. The name Strunal was used for the company between 1992-2016, as you can read on their website: Invalid Link Removed

The name Josef Jan Dvořák was used for their export models.

I can't help with deciphering the pencil numbers though.

I got one of their plywood basses, it's a good reasonable bass.
