Model J wiring help

I´ve recently purchased a set of model J jazz pickups for my bass. My plan was to wire them up with 2 volumes and one tone knob with push pull functioning as a series/parallel switch.
I've wired it up using the following schematic.


The problem I encountered is that it doesn´t work as intended. I triple checked every connection and made sure the grounds are grounded properly. When I plug in the bass I hear a humming sound indicating a faulty ground. The pickups make no sound whatsoever, only the bridge pickup is responsive when both volume knobs are pushed down and the tone knob pulled up.

I hope one of you can help me resolve this issue.
