The schematic is confusing but the heater voltage is already rectified (full wave) to DC.
My thoughts are a ground loop between the output and the power amp (a 1:1 audio isolation transformer is the proper and safe way to deal with this), or possibly noise getting into the V5 stages (via a defective tube, noisy heater supply, noisy plate supply or possibly an internal ground loop or poor ground current steering.

Sorry, I'm in my work van, looks at a glance like something you'd connect inline with the output to the power amp, I did not have time to read the specs or anything. Just saw 1/4 inch in and outputs.
Notch filter came to mind.
This preamp has a transformer balanced line level output. Perhaps that could be used?

Search google on audio isolation transformer, it's interesting stuff.
That was my first thought, but the same conclusion... though it could be converted to line level. It would be "necessary" in this case for the power amp to have balanced inputs.

That's what I thoughtV5 (both halves) are out of the picture when using the balanced output. It's also before the master volume control.
