New David Gage Lifeline not working!

Hello,
I just had a brand new David Gage Lifeline pickup installed on my bass and it does not seem to work correctly! It is installed properly on the non threaded side of the bridge post. The guy who installed it even used to work at David's shop. When it is plugged in I either get a crackling noise or I have an amp at max volume and some sound comes out but it's woofy and kind of feeds back. Sometimes it doesn't make a sound at all. Has anyone experienced this before? Also if someone has this can you tell me if your amp has to be at max volume to use it? I thought this was a high volume output pickup. I am planning on returning it but I just want to check with some other people about it before is swap it out. Thank you!
 
take it back (still installed) to the guy who put it in, maybe he can fix the issue.

also, have you tried different cables and even a different amp to make sure the problem is actually the pickup?
