Schematic for passive tone control?

I know it's very simple, but I can't find a schematic for the traditional passive treble roll-off control, anywhere on this site or with Google. I guess the term is so ubiquitous that it's impossible to wade through the millions of pages about Stratocasters.

Anybody have a schem, or can anyone just explain it to me? I think it's just a cap and a pot, but how exactly does it need to be wired up?
