I have a Jawbone bluetooth earpiece. I like it a lot because it sounds better than my phone and has noise cancelling mumbo jumbo built in so that I can be somewhere noisy and still have a conversation without irritating who I'm talking to (with the environment - it doesn't help me out personally, sadly).
It also makes it easier to do things while sitting in on conference calls, which is mainly why I got it.
I do take it out when I'm not using it. Those things make people look like they have a wang stapled to the side of their head. I can't believe it's turned into such a status symbol.
