I would think a more prevalent problem is misusing a grave accent when you really want to use a ...um ... what the hell would you call it? A Start-Single-Quotation mark?

Y'know, like when a quote is embedded within another quote; e.g.,

Johnny said "If I ever hear Sarah tell me `You have a tiny pee pee' one more time, Imma go ballistic, yo!"
