I've seen multiple books before with the off center cover, both Harry Potter and others. Of the ones I've seen the opposing cover is cut flush with the paper, so it looks like it was done on purpose. Either that or it's just a common mistake.

They're able to pierce areas of the bark to get at the sap carrying cells underneath. They usually don't do a whole lot of damage to mature trees, but the honeydew they secrete can cause sooty mold to grow and attract all those bees and ants looking for some free sugar.

The aphids are most likely a species in the genus Cinara. Black, long legged, and they feed on conifers.

Wash the aphids off with a garden hose and treat with insecticidal soap if need be.
