Points about dress code are really only valid with respect to genres, venues, cover, original, etc.

If a bunch of 50-60 years olds are out listening to a bunch of 50-60 year olds playing classic rock at Bob's corner bar, they probably don't care about cargo shorts and baseball hats.

You can now make the same comparison to each respective "scene" in your demo.
 
Well, here was little bit of everything style wise in one band lineup.

I wish they would have added something about volume I stopped going to local live music venues because bands are getting louder and louder. I see bands where everyone is wearing IEMs or everyone has a monitor including the drummer and the stage is no bigger than a bar stool playing for a crowd of twenty people. I see full stacks and SVTs with 8x10s blaring so loud it's just a distorted mess so many bands simply can't manage their sound? maybe it's a generational thing and I'm out of the loop.

The only thing I'd disagree with is:
4. Image is actually important. Cargo shorts are for dads at a barbecue. Not for musicians on stage.
Yes you must totally dress the part for being on stage; however I'd bee a little careful about citing specific examples since everyone's context is different. If it were me I'd say that under no circumstances should a man wear Hawaiian print shirts, ever, period, with the possible exception of actually being in Hawaii. However they seem frighteningly acceptable for old guy cover bands. Cargo shorts, especially camo print ones, are perfectly acceptable for people playing rock or anything higher on the Clark Scale of Heaviness.

Here's something to add to your list of things local bands don't get:
18.) Paid.
