It's Eb Rhythm Changes, but with a very interesting(!) harmonic treatment of the Melody in the A sections, and a "DIY" melody on the Bridge.

Beautiful job. It's nice to see beautifully hand-copied music. There's a written bridge to this song on the earlier version with Clifford Brown.

Yes, I did hear the CB version with the composed Bridge, but I thought I'd replicate Elmo's original version.
(I should really check out and write-up the CB Bridge, as Elmo H. is the pianist on that recording. So many tunes/so little time!)
