Are you wanting something fancy, like wood or abalone inlay? For that kind of thing, you should post over in the Luthier's Corner. Lots of very talented people over there that do that kind of thing.
If you just want something simple, you can order a custom made decal. Just do a google search for "guitar waterslide decals", there's a bunch of places that'll print them for you. Lots of people here have got them on ebay.
