Help Identify Bass Preamp

I know that it is most likely a Demeter VTBP-201DB Bass Preamp, but it could also be a VTBP-201DBL or VTBP-202. The logo seems unique compared to other ones I've seen.

The company was Innovative Audio until 1990.

As I remember, and I am happy to be corrected...

How to tell which you have:

1. DI uses the Jensen DBE transformer (you have this). This is the DB suffix in the VTBP-201DB
2. DI and Effects loop. VTBP-201DBL
3. Crossover. VTBP-202

As for what year, I can't say other than pre-1990. They changed the graphics multiple times while it was Innovative Audio.
