In the fingering chart, G string, III1/2 position it shows C#(1), D(2), E(4). Shouldn't it be Eb? All the other strings read 1/2 steps up from finger 1 in the III 1/2 position.

Sure seems like Eb is right to me, also. What page of what edition of Simandl has this chart? I'll check mine as soon as I can find it.

BTW, that's a really old edition you have there, like pre-1960. I have the blue cover english-only one, and in the forward, Mr. Sankey explains how they simplified the naming of the positions, to remove all those 'half positions' except the first one.

No kidding? In Simandl it's now just h.P., I, II, III, IV, etc.? Or are they using Arabic numbers now, too? How long have the modern Simandl books been doing this? Makes good sense.
The version with the blue cover is the International Music editon in which Stuart Sankey changed the position numbering system to eliminate all of the half positions except the first one. The Carl Fischer editions use the standard Simandl position nomenclature and does have a typo in the fingering chart. I've always noticed that.
