We're giving wiseguy answers because your question is meaningless.
There has never been an SPL test of every amp/cab combination out there. So there is no measured "loudest," period.
There is no "best-featured" head. Sure, there are heads with different features. For example, the EQ on an SWR, Eden, Mesa, GK, Genz, etc are all different. Is one better? You decide.
"What's the best car?" Is it a Ferrari? Not if you're carrying cargo. Is it an SUV? Not if you're racing.
What's the best meal? Who's the best bassist?
There is no answer to your question. "Best" is a combination of all the factors that differ. Sound, size, weight, price, loudness, availability, appearance, reliability, serviceability. You pick the requirements you need.

A Crest CA9 (plus a preamp of your choice) driving two Bergantino 4 ohm NV412 cabs would be very loud if you needed it to be. 
It might be difficult to find those cabs used...
Be sure to budget for a large vehicle, back surgery, and a hearing aid.

On the subject of loud amps, what has been heard of the new Bugera "Nuke"? I'm sceptical of its specs and quality, being made by Behringer, but some guitarists swear by Bugeras.....
Has anyone here tried one?
Specs claimed: 3600W "Bridged Mono" (Is this a stereo amp? They do not say in the MF catalog), SVT-styled EQ (3-Bands, switches for "Ultra-low" and "Bright"), additional 9-band graphic EQ, Adjustable compressor, Switchable limiter, $600.
If this is halfway decent sounding and the specs are halfway accurate, I would buy one of these for sure.
EDIT: Checked out more detailed specs, it is in fact stereo and has many more features than I described, pretty much anything I could concieve to put on an amp. There were 2 reviews on MF, one said it didn't work, right out of the box, and the other one said it was perfect and that Bugera was a "Fighter jet made of biceps". This review was posted twice.
EDIT #2: Looks like they have another amp, a 550W all-tube model. Dunno specs.

Yeah it doesn't take much around this joint to be left for dead, bleeding from every orifice and feeling really really stupid.

NAAAAW, those are PEAK RMS watts, a totally new meaningless measurement done by guys in white lab coats with equipment non-standardized anywhere but in the Bavarian Alps.
They also measured it at 162 line volts on the USA model.
In RMS real live respectable watts on standard house power (RMSRLRWOSHP) it makes 87 watts per channel into 2.78 OHMS for 30 seconds.
