First, it's an OUTPUT jack.
Second, any 1/4" TRS jack will work, so long as it's the right size.
Third, you would be better off replacing the jack with a good open-frame Switchcraft jack, rather than those cheap plastic ones.
But, doesn't this particular jack shut off the battery when you pull out the plug?
There's like four posts to solder to.
The standard method of wiring an active bass is to use a TRS ("Stereo") jack.
The signal goes to the tip terminal, the sleeve terminal is a ground, and the negative terminal of the battery goes to the ring terminal, such that inserting a mono cable shorts the ring and sleeve together to power the preamp.
There should not be any additional terminals on your jack, and if there are, they would be unused.
