Alto TSSub18 smack in the middle laying on its side to reduce the profile, or is that a problem?

I don't see a problem with putting it on its side, but would still look strange and take up prime real-estate.

Ideally, you want your monitors right behind your mics to put them in the rejection zone of the mics and reduce feedback.

run both the L and R lines out of the mixer into the sub, run a single XLR line per main from the back of the sub, and run my monitors from the 1/4" control room / monitor ports on the mixer.

This would be a correct way to run the cables.

True. However, ideally you want your pair to be next to each other. If you put one on one side of the stage and the other on the other side you will make the bass very loud in some parts of the audience, and very thin in other parts due to phasing issues. This is the reason I only use one (and for budget reasons too)

Playing outside generally you need even more since you can not get any room reflections.

Yes, outdoors is the place where you need all you can get.

Well for one the 118P is a subwoofer the 115p isn't.

Placing your sub in a corner will give you a free +6db spl. That may make the 1 sub more viable. If you don't group subs, they need to be 40-50ft apart to avoid cancellation IIRC.
