Practice Schedule

Hi

Right now I'm in the process of planning out a practice schedule for my fall semester of school (music college). I was wondering how to go about doing this? I've already written out a few pages of what to practice (scales, chords, reading, etc. with notes to myself), but am not really sure on how to go about crafting a "schedule." Should I actually plan out times, or just say I'll spend a certain amount of time on each item? Also, what would be good 'subjects' to follow each other?

The 'subjects' I've got laid out are:
Scales
Rhythm
Reading
Improvisation
Transcibing
