This is what I use:

This will give you white on black. I made up the logo I wanted on a Photoshop-type program, saved it to a flash drive, and took it to the UPS Store. They'll print it from the drive to their high-dollar laser printer for cheap ($3.25 out-the-door cost for me).
A few things to keep in mind if you go this route:
- The printed black will, in all likelihood, not be a perfect match for your paint, so you'll want to trim it fairly close. X-Acto knife to the rescue.
- Make SURE your print size is actual size for your decal, to avoid any Spinal Tap-esque scale issues.
- You'll want to apply 2-3 coats of spray clear coat to the sheet after it's printed (but BEFORE you cut them out). This keeps the ink from smudging during application and helps expedite things when you clear coat the headstock later. The decal is fragile and will have to have clear coat over it to protect it.
- Since the decal media is white, it will show on the edge of the decal after you cut it out. I take a sharpie to the edge and let it dry before it goes in the water. Problem solved.
EDIT: Make sure the squints at the UPS Store know to set their printer for "photo paper." They should be able to figure it out on their own, but I wouldn't leave it to chance.
