I think it may be a John Birch custom. Birch made some SGs for Tony Iommi and pickups for all his guitars.

edit: it looks like a les paul double cutaway and hamer made guitars that looked like that.. also hagstrom..

I remember reading somewhere that when geezer's pbass got destroyed he started just getting custom john birch and jaydee basses.

I think some were even rick copies, how'd they get away with that?
