I was restringing and came across something I'd never seen before. I wrote to Ernie Ball Customer Service about it and they responded immediately.

A few days later, I got this in the mail.

Big thanks to Ernie Ball Customer Service for taking care of this so quickly!

I once bought a 10-pack of acoustic guitar strings (D'Addario EJ11's), but only used 1 or 2 sets before buying a bunch of other string brands to experiment with. Then when I finally got back to using the other strings (that I'd bought 10 sets of), I discovered that 4 of those (cardboard) string packs were totally empty.
When buying multiple sets, always inspect ALL of them upon arrival.

I recently received a sealed pack of strings that only contained a D and a G. I took it up with the vendor who made it right immediately (and who, I guess, must have taken it up with the manufacturer) but I couldn’t help but wonder how a half-empty pack of strings left the factory.
Because someone ordered a new set, took 2 strings out they needed and returned it. They didn't check and restocked it. Happened to me on a truck part. I'm sure this happens more than you think across all industries. Can't afford to pay someone to verify returns.

Similar story: I got a set of BEEDG Cobalt flats from Amazon a year and a half ago. Figured it was fluke, processed an exchange, and the replacement set had the same issue. On mine, the envelopes said 80, but there were really 100s so I didn't catch the issue until the string didn't fit in the A nut slot. I didn't want a 3rd set of BEEDG strings so contacted Ernie Ball, and they immediately sent me 2 correct A strings. At least their customer service is on point.
