NCJZ Bass = (N)icks (C)ustom (JZ) jazz Bass

You can also just simply remove the large SQUIRE letters and just keep the small fender letters followed by N.J custom
or NCJZ Bass

There are some vendors on here. If you have an colour inkjet printer and some decal paper, and some Krylon or Rustoleum Painter's Touch clear gloss, you can make your own.
