That’s what I was thinking. That it might have been defective. I changed my strings 3 months ago.Had that happen on an E string many, many years ago. It was a really old string. I think this can happen if the strings are defective, too. Even if they aren't old. Bummer.
I play up and down strokes alternating between my index and middle fingers as well. My action is fairly high so I can dig in and get the sound I want.

I play a four banger as well. It’s just tuned BEAD with heavier gauge strings (.125)Oh man, now it's real! That sucks, I broke an E (only 4 bangers for me) the other day, but that was a defective string that let go while I was tuning up.
