When do you think a matching headstock does not look good?

I’m not a fan of matched headstocks in general, but especially when it’s a Fender-style bass with a maple fretboard.
Generally all the time. - There are some exceptions, but in general, I think the core wood should just be the color of the headstock.
Also, kinda on the subject, not a fan of blank headstocks from custom neck shops either. I think whoever the maker, should offer a factory applied logo decal as an option. Especially when it's an FSO headstock. Blank ones just look naked.

If it clashes with the fretboard material.
Not to waffle, but I did say there are some exceptions. That would be one...
It just depends on the vibe of the bass...
I would never want a sunburst headstock to go with a sunburst bass. 
Or, a tort headstock to go with a tort PG. 
Or, a tort headstock to go with a tort PG. 
I don't like matching headstocks with maple fretboards. Somehow a rosewood or ebony board ties the headstock to the body, imo.
I think the matching headstock is always a feature that makes a bass look complete.
Whether or not I like the finish on the bass is another matter.

If the body of the bass is ugly and the headstock matches, well....
Once in a while burst headstocks look good but most I'm not a fan of. I think the headstock area is a bit small for a burst.
Bought this in March. Not that big of a deal but I wish the headstock was gloss black.
It depends, I havent set any rules. Some I like some I dont. I do generally agree with the idea of not with a maple fret/finger board but Im sure an exception exists. I like the Ibby matching, also like it in all black/matching. I like the matching Stingray fretless, would not like the Classic white Ray if it was matching.
Only if it was painted some obnoxious color to match the body, I generally like matching headstock, it just ties the whole thing together for me. Punches out the logos nicely too.
Never looks good. I hate matching headstocks. Time, effort, and care into matching the body? Absolute trash, imho.
As mentioned a few times above, maple fingerboards.

Back in the early 60’s painted or matching headstock signifies a low end knockoff instrument.
So as a bass player who started back then I still think it makes the instrument look cheap.
Only the Beatle Bass pulls it off fairly well. IMVHO
