First, I'd try a jumper cable from 'preamp out - power amp in'.

That is true, too. And regardless of how old it is as well.
Tubes are unpredictable like that. Always have been.
Could it also be the quality of the electricity provided by the venue? I'm asking as I heard many times Class D amps a very picky about the quality of the electricity.
It’s a possibility, have you tried the rig elsewhere and gotten better results?
Thats exactly what I want to do. But I don't have the opportunity yet.
However, as I mentioned earlier, I didn't face any issues when I used it as power amp last summer.
I believe that Rev H started in 2014 or 15. Not exactly sure on that one. You have to open up the amp to read the revision number on the motherboard, though you can date it by serial number (although this gets a bit confusing as the date digit moves depending on the product, and there are several letters in the serial number). But generally, the date digit is between the 9th or 12th digit, and it's a letter:
A: 2008
B: 2009
Etc. Takes a little bit of reasonable deduction and knowledge of product time frames to figure it out, but Ampeg CS can tell you when it was made and what board it should have if you have a specific one you're interested in.

About the issue that I faced a several weeks ago, I think the electricity provided by the venue was actually the cause. I bashed my SVT 7 with my two SWR Goliath III cabs, and I couldn't reproduce the issue.
It was a little stage and my guess is the lights were pumping most of the power.

Finally got my amp opened up and took some pictures to see the version letter etc. Does this mean I have revision C and I am susceptible to the flaw? And if so, is there still anything that Ampeg (aka Yamaha) will do to fix this for me?

Your amp has a REV H main board, which is the latest and the one that mattered. You should be just fine.
You may be looking at the housing’s revision sticker. You’d need to look at the main board inside the amp. Rev H is what you’re looking for in there.
