Pieter Bruegel the Elder - The Triumph of Death. Used for Black Sabbath's Greatest Hits, but not for long.

Also, just about every Scorpions album cover from the 70s. Including the banned one with a naked 14 year old girl on it. Classy.
Unsane has a few that I shouldn’t or can’t post. Their 1991 album is on my list of unforgettable covers.
Carcass and Cannibal Corpse get some honorable mentions. Possibly leaning to more shocking or than creepy, but I’d say both.
Reek of Putrefaction is a strong contender.
Butchered at Birth and Tomb of the Mutilated are on my list.
I wouldn’t suggest searching for these if you don’t know them. Definitely not at work.

wow that sounds so much like sabbath....i like the vocals a lot..that early 70's stuff is great..i'm letting this whole album play out with bass in hand
The new Havok album artwork is kinda creepy. I think it's pretty gnarly.
That first choon is like Dio doin', "Immigrant Song".
The second is like Whitesnake doin', "Highway Star".
