FAIL!! Schaller Strap Locks

We never found out what exactly failed.
Did the part on the strap come unthreaded? Loctite is your friend there.
Did the spring loaded pin somehow pop out? If so, the U shape should have still held the pieces together.

You want your schallers to never come off? Install them with the included longer screws (so they grip new wood), face the lock so it's cradling the strap pin when installed (to hold the weight), and tighten the bloody nuts down with a socket! You wouldn't install a bridge with finger tight screws, why would you install your locks like this? Don't rely on your fingers... As for Dunlops/Fenders being better than Schallers. Nah. The Schallers have the ability for the lock itself to hold the weight of the bass. The Dunlops rely on a pin in the middle to hold all the weight. No thanks.
No these are the locks that came with the bass. And yes the nut loosened just enough so as to relieve tension on the lock.
I can barely play bass! Didn't really pay attention in typing class! Too many girls!!

I've never had an issue with Schallers and have them on everything I own, even my acoustic electric. Before every gig I always make sure the bolts on the strap ends are tight and make sure the underside strap portion holds the end pin like a U verses facing down.
Dunlops are okay, They just jingle if you have a thin strap and the install screws could be made from a better alloy.

I have used Schaller strap locks for at least twenty years and love them. I have them on every bass and every strap I own.
Guys... Sometimes "stuff" just happens. Please understand that these locks were recently installed, maybe a week old. They were installed using tools, the correct size open end wrench. No I didn't use Loctite. They didn't have time to work loose.
All I am trying to say is never fully trust anything mechanical. You bring a backup bass with you to gigs don't you? Extra cables, extension cords, power supply, fuses, picks, strings, etc.? Murphy's law.
If this weren't a bass forum I would tell the story of my brand new Z06! Love the car, but had QC issues!

This is definitely not common. There are probably a lot more basses hitting the stage due to not using some sort of locking mechanism. I've been using Shallers and Strap locks for decades and have never had this happen.
This. 14 years for me. No failures, no scratches, no "teetering" when leaned against amp, no squeaking... just solid reliability while working as described... once properly installed.
I've had Schallers for about 6 months on my Jaguar bass and a cheap Ibanez SS. The only issue I've had is the nut and washer came off the Ibanez, but the strap stayed on with no problem. Oddly enough, this bass sees minimal use so I'm not sure how or why it would have come off of this one and not the Jaguar. Guess I did a bad install.
I think the reason you're getting a lot of the types of responses you're getting here is that your OP suggests that the product was defective and the evidence doesn't seem to be pointing in that direction. I'm still not saying you're wrong, I just don't quite get the nature of the failure.
I'm trying to figure out exactly what you mean here, so I can get a better picture in my head of what, exactly, happened.
Man, that's a bummer sorry to hear about that. The only thing I have noticed is on my Schaller strap locks I have a black leather strap and leather stretches and dents, if you don't tighten the nut on the strap locks every once in a while it will get loose. You can check it once in a while or get a lock washer under the nut and still check it from time to time to be safe....
Strap locks are kinda like bindings on a snowboard (to me) and shud be double checked and re-tightened every second tuesday of a month with an "r" in it or every full moon or right before you use them.
I wasn't the one who installed them but regardless, the stripped wood was the least of my worries with those strap locks. Between the self-loosening nuts, the moving U cups and the irritating noise they would make, those locks were way too high maintenance for my liking.
I've used them since they came on the market. I put a little Loctite on the threads during installation. I've never had a problem.
The OP is not explaining it very well, but from his description there are two possible answers: the nut/washer came off, possibly due to poor manufacturing as found by kzr750r1(loose threads) or the plunger button came off the locking pin.
