Strange Issue with Headphone Amp Setup

Hopefully this is the right forum for this.

I am close to perfecting a headphone amp setup, but I can't solve one annoying issue. When I use a headphone extension cable, audio is only output to the right ear.

The setup I am trying for is:

Amp (TRS out) -> 1/4" adapter (TRS) -> extension cable (TRS) -> TRRS to TRS adapter -> TRRS headphones

When I attach the headphones directly to the 1/4" adapter, the audio output goes to both ears as expected.

What do I need to change to get this setup to work?
